Use this form to retain our services for your closing, such as for the purchase of your new home or your refinance.  Not all information requested in the blanks needs to be provided, though I must have, at minimum, some means of contacting you.  All blanks are optional:  feel free to provide as much information (or as little) as you want.  For example, if you are refinancing, many of the blanks are not applicable, such as realtor and seller info.  When you have entered all information you are going to provide, click on the "Submit" button at the bottom of the form.

I will not share any information you provide on this form with anyone else (other than, to the extent necessary, with the other parties involved that you identify below).  However, please use use some other medium (telephone, in person visit, etc.) to give me information that is confidential or secret.  Note that no Attorney-Client relationship or privilege will arise from your sending this form or other email to me; that does not arise unless I respond back to you and agree to handle your closing (or other legal matter).
